Six Pillars, One Mission

What We Do

Focused programs that close the biggest gaps in care — built around what each community needs most.

Specialty Care Solutions

Expert diagnosis and treatment delivered where specialists are scarce.

Bridging Rural Care Gaps

Linking remote areas to the services they can't reach on their own.

Global Health & Disease Elimination

Fighting HIV, hepatitis, tuberculosis, and malaria in communities at home and abroad.

Community & School Outreach

Meeting people where they live, learn, and gather.

Health Literacy & Education

Culturally responsive prevention programs that build lasting knowledge.

High-Need Populations

Neonates, children, medically fragile, and low-income families the system too often overlooks.

Public and Global Health

One Mission, Local to Global

The same mission behind our specialty care and health literacy work extends to public health and disease elimination — advancing the health of underserved communities, both here in the U.S. and around the world.

Public Health & Disease Elimination

Bringing our specialty care and health literacy to the fight against transmissible disease — prevention, testing, and expert clinical care for HIV, hepatitis, and other transmissible infections in the communities we serve.

Global Health

Extending that work worldwide — tackling tuberculosis, HIV, malaria, and more through strategic partnerships that bring expert clinical care and innovative solutions to underserved communities in the U.S. and abroad.

Our Model of Service

One Connected Model of Care

Reaching high-need families where they are — at home and around the world — through partnerships that bring targeted care, expertise, and education to every community we serve.

1

Identify & Reach

We find high-need, often-overlooked families — neonates, children with special needs, foster and immigrant communities — and meet them where they are.

2

Connect to Care

Real-time telehealth links bedside teams to subspecialty expertise — cutting transfers and bringing earlier diagnosis to sites with no specialist on staff.

3

Educate & Prevent

Health-literacy and prevention programs equip families and frontline clinicians with the knowledge to keep communities healthier for the long term.

4

Sustain & Partner

Hospitals, clinics, schools, health departments, and global health partners share one mission — bringing durable, expert care to communities at home and abroad.

Global Health & Disease Elimination

Our Model to Eliminate Hepatitis C

Hepatitis C is now curable — yet only about 1 in 5 people living with it are diagnosed. Our test-to-treat model closes that gap in the communities we serve, advancing the global goal to eliminate hepatitis C by 2030.

1

Screen & Find

Community-based, point-of-care testing.

2

Link to Care

Connect newly diagnosed people to expert clinical care.

3

Treat & Cure

Direct-acting antivirals cure — working collaboratively with primary care and safety-net providers.

4

Prevent & Sustain

Harm reduction, education, reinfection prevention.
